Supplemental Form
When you apply to the 2011-2012 BEd program at UOIT, you must complete a TEAS application form and a UOIT Supplemental Form. Both are obtained through the Teacher Education Application Service (TEAS) at this website: http://www.ouac.on.ca/teas . The Supplemental Form can also be downloaded at the bottom of this page.
The supplemental form includes a personal profile and two references: one personal, and one academic. The Office of the Registrar at UOIT must receive your supplemental package by the same date the completed TEAS applications are due. (December 1, 2010) Please see the "Admission Requirements" and "Admissions Process " sections of this website for information regarding the personal profile and reference letters.
Applicants who accept an offer to UOIT will be required to submit proof of a TB test and a criminal reference check.
NOTE: You must complete the online application at the Teacher Education Application Service website BEFORE you complete and send the supplemental form.
The supplemental package for the 2011/12 application will be available for download once the application officially opens via the OUAC website in mid-September, 2010.
